1. Login to Opus EHR
2. Navigate to the Patients tab
3. Click on the Patient's chart
4. Click on Outcome Measurement Tools
5. This tool will give you a graph view of any auto-scoring assessments that the patient has completed. To view the full assessment of any score, hover over the score and choose "Open Result"
