A Patient's Guide to Completing Documentation Through the Patient Portal

In this article, we provide a brief step-by-step guide for how to complete documentation through the Opus patient portal.

Step 1: Check Your Email

  • You will receive an email from Opus Team. This email contains a button to access the patient portal. Ensure to check your inbox, including spam or junk folders, for this email.

Step 2: Click on the Access Patient Portal Button

  • Open the email received from Opus Team and locate the button labeled "Access Patient Portal".
  • Click on the "Access Patient Portal" button. This action will redirect you to the Opus patient portal login page.

Step 3: Navigate to the Home Screen

  • After logging in successfully, you'll be directed to the home screen of the patient portal.
  • Look for a section labeled "My Pending Documents". This is where you can find documents that require your attention.

Step 4: View Pending Documents

  • In the "Pending Documents" section, you'll see a list of documents that are awaiting completion or review.
  • Click on 'start' next to the document you wish to complete or review. This will open the document for viewing and/or editing.

Step 5: Complete the Document

  • Fill out the document with the required information. Depending on the document, you may need to input personal details, medical history, consent information, etc.
  • Ensure to review the document thoroughly before submission to avoid any errors or omissions.

Step 8: Submit the Document

  • After completing the document, look for a button labeled "Complete and Submit".
  • Click on the "Complete and Submit" button to finalize the document.

Step 9: Confirmation

  • Once the document is successfully submitted, you may receive a confirmation message or notification acknowledging the completion of the document.
  • You can revisit the patient portal at any time to track the status of submitted documents or to complete any remaining tasks.
